Patient Case: Lichen Sclerosus Treatment Before and After

This patient is a physician and pediatrician who had been diagnosed with lichen sclerosus more than seven years earlier. Her symptoms included cracking and bleeding of the vulvar and perianal skin, along with pain, irritation and persistent itching.

Initially, the condition primarily affected the perineum and the area surrounding the anus. Over several years, however, her lichen sclerosus progressed and began affecting the clitoral hood and surrounding vulvar tissue.

Her symptoms became significant enough to interfere with everyday activities. Walking and exercising could cause rubbing and irritation, and she was not sexually active because of her symptoms and pain with intercourse.

Previous Lichen Sclerosus Treatments

Before coming to Miklos & Moore, the patient had tried several therapies for lichen sclerosus, including:

  • Clobetasol steroid cream
  • Estrogen cream
  • Intravenous immunoglobulin (IVIG) therapy

She reported that her lichen sclerosus symptoms tended to improve and recur over time, with periods of increased inflammation, cracking and discomfort.

Radiofrequency Microneedling Treatment

After researching additional treatment options, the patient became interested in radiofrequency microneedling (Morpheus) combined with platelet therapy and Amdrop™ injections.

She underwent three treatments, spaced approximately four to six weeks apart.

Lichen Sclerosus Treatment Results

Following treatment, there were noticeable changes in the appearance and integrity of the affected vulvar tissue, as well as improvements in tissue pliability. More importantly, the patient reported a dramatic improvement in her lichen sclerosus symptoms.

She is now able to walk long distances, exercise and enjoy activities that had caused significant discomfort and irritation for years.
